Medical Directorship
The role of the medical director in the post-acute and long-term care setting is a critical one. It requires specialized physician knowledge, training, and experience – as well as medical management skills that can effectively balance patient care and business success.

Our medical director services are available to our partnering organizations across the care continuum, including skilled nursing, inpatient rehab and assisted living care settings. Our Medical Directors focus on working with the post-acute operations and clinical teams to streamline processes utilizing clinical best practices, attend QAPI Meetings to assist team with improving performance of key indicators. Medical Directors provide ongoing education and training with staff to ensure the patient satisfaction and quality outcomes.
Primary Care
Daiya Healthcare introduces a unique model of care, offering streamlined processes and evidenced based medical treatment. Our Internal Medicine Providers are onsite daily at our partnering facilities, collaborating with staff to deliver the highest quality of care possible. The primary care physician supports new admissions, history and physicals, advanced care planning, regulatory visits, care coordination and care transitions/discharges from facility. With direct provider oversight we are able to treat a wide variety of acute and chronic conditions in-house, reducing hospitalizations. Our dedicated team of post-acute care providers provide 24/7 call coverage to address the ongoing patient care needs.
Specialty Care
The Specialty Care Team at Daiya Healthcare includes Board-Certified Physicians, Nurse Practitioners, Physician Assistants, Pharmacists, Registered Nurses, and Medical Assistants. Along with being trained and experienced in their respective fields, all Daiya Healthcare specialists undergo a proprietary residency training program and on-site job orientation individualized for each care setting. At Daiya Healthcare, our goal is to provide accessible healthcare services 24 hours per day. Telemedicine is an integral part of the post-acute care services that we provide. Telemedicine offers a variety of benefits to the patients in our partnering facilities, including:
